AWARD
Scholarship Deadline:

ADMINISTRATION
1. The Scholarship Committee shall serve as the Administrator of the Scholarship Fund.
2. The administrators of the scholarship fund shall vote on the amount of the scholarship award.
3. All award funds will be paid directly to the scholarship recipient(s) within 90 days.
4. Our goal is to encourage recipients to return to Fort Wayne, IN.
CRITERIA – Applicant:
1. Must be enrolled in a health science program at an accredited college or university.
2. Must maintain a grade point average of 3.0 on a 4.0 scale.
3. Only eligible to receive scholarship dollars for a maximum of four years.
4. Must demonstrate leadership potential as evidenced by information included in the application.
5. Must submit documented community service involvement.
6. Scholarship awards shall be available to students from Fort Wayne, IN that are enrolled and accepted in a health-related field.
PROCEDURE
1. Submit completed application (below) to the scholarship committee one month prior to scholarship ball. Applications must be submitted online.
2. The following required documents shall be sent via email to: aahafortwayne.org@gmail.com once you have submitted your application. Please include the Subject line as follows: First name, last name and year (example: John Doe 2024).
a. Most recent transcript.
b. Letter of recommendation from academic advisor or teacher.
c. Essay describing educational goals.
d. Proof of current enrollment in an accredited health-related school.
e. Documentation of community involvement and leadership ability.
f. Your attendance at the scholarship ball is requested.
